What is adductor tendinosis?

Adductor tendinitis is a medical condition that is caused due to injury to the hip adductor Tendons. As a result, it causes pain in your inner thighs, the groin. It shows several conditions develop near the tendon, which results in the change of molecule structure of the tendon. The adductor has 5  muscles which are categorized into long and short adductors.

The long adductors are Gracilis and Adductor Magnus, and the short adductors are Pectineus , Adductor Brevis, and Longus. The long adductors attached to the pelvis extend to the knee, and the short adductor also gets attached to the pelvis. The adductor muscle helps to stabilize the hip, keep the thigh together, and rotate the upper leg inwardly.

The muscle extends from the lower Pelvic bone and gets attached to the lower femur area. The muscle plays an important role in standing and walking. The muscle has two parts: the adductor part extending from the inferior pubic ramus and ischial ramus, attaching to the line aspera of the femur and medial epicondyle. The second one is the hamstring from ischial tuberosity to medical Supracondyle and Adductor tubercle.

What is the cause of adductor tendinosis?

The main reason for adductor tendinosis is the overuse of the adductor muscle. It is caused due to sports and activities like martial arts, gymnastic, football, running, horseback riding, and many more. In this case, the muscle randomly stresses and changes its direction, which results in a serious cause.

Sometimes we confuse tendonitis and tendinopathy because both diseases share similar symptoms. Tendonitis is inflammation of a tendon of muscle which causes pain, whereas tendinopathy doesn't have any inflammation. It decreases muscle strength and causes pain. It is more like a degenerative change in tendon.

What are the symptoms of adductor tendinosis?

The symptoms of adductor tendinopathy are as follows -

  • Deep ache in the groin after some physical activities.
  • Weakness of adductor muscle
  • Tender-to-the-touch
  • Pain in the groin area
  • Increase of pain during adductor construction. For example: - moving legs and squeezing legs.
  • Difficulty while walking and running.
  • Night and morning stiffness

Diagnosis of adductor tendinosis

Adductor tendinopathy is diagnosed by taking a patient history of groin pain and pain near hip muscles. After taking the history, the doctor may advise to have an MRI to rule out other conditions.

There are many differential diagnoses of the problem. Some are iliopsoas bursitis, nerve compression, stress fracture, avulsion fracture, snapping hip syndrome, and inguinal hernia.

You can also do an ultrasound for confirmation of tendinopathy. It helps to determine tendon thickness, tears, and dislocation.

Treatment of adductors tendinosis -

  1. The treatment consists the proper rest. Physiotherapy is the best for a patient who is suffering from adductor tendinosis. The doctors may recommend avoiding other physical activities because it increases the groin pain.
  2. The doctors prescribe anti-inflammatory drugs to reduce the pain and swelling. It may be effective in the early stage, but later it helps in recovery. A therapist applies electrotherapy, like ultrasound, to reduce symptoms.
  3. Massage Therapy is also done for the patients. It improves muscle condition and helps release lumps and tight knots.
  4. Stretching and strengthening exercises are recommended for speedy recovery in the patient. It helps to get rid of inflammation. If pain occurs during the exercise or after exercise, then stop the exercise and take proper rest. When pain becomes less, then exercise should be done again. The main aim is to increase the load through the injured muscle.

Prevention

To stop the development of adductor tendinopathy, the patient has to work on factors like improving strength and muscle coordination. It takes time to recover completely, so during the recovery, the patient has to take care of everything which affects muscles and results in pain. With the help of exercise, the person develops muscular strength and stability near the pelvic areas and groin. Mobility and muscular support also help to overcome the problem. The patient is advised to do proper warm-up and sports-specific training.
